Initial Assessment Team (IAT)

What are we?

The Initial Assessment Team (IAT) is the first point of contact for adults and older people who feel they need support to live as independently as possible.

What do we do?

This team can provide advice and information. If you think you need practical help at home, the team can arrange for an assessment of your needs.

Contact this service if you are a Trafford resident aged over 18 and you are finding it difficult to look after yourself and live in the way you want because you:

  • Have a physical disability or injury
  • Have lost some or all of your hearing or sight
  • Have a learning disability or mental health problem
  • You are generally frail
  • Your are a carer who needs advice and / or support with your caring role

What happens when I contact the IAT?

We will ask for some basic details and usually ask what you think your needs are.

If you are new to Social Services, we will identify priority needs and arrange to visit you at home, if necessary, to work out a plan of action. You are welcome to have someone with you when we visit. We can also arrange an interpreter if you need one.

We provide a short-term service aimed at getting people the help they need quickly. If your situation is complex or likely to be long-term we will arrange for a specialist team to work with you.

Is there a charge?
